Creating a Form is easy but getting it work by integrating all form related components is a tedious task and it doesn't change much with the number of forms you have.
Form components such as validations, storing form data in database, sending mail to the users( in cases such as contact form) Form Reader works like a charm for you.
Use the admin email address set in Settings->General as the email address from which WordPress sends email. The plugin has no settings, does not alter the database and only uses hooks to achieve it's goal. It also should work perfectly fine with Multisite installations....
